Position Overview
The Health Coach serves as a vital bridge between our senior patients and their clinical care team, implementing our nationally recognized clinical model – one that has been published in leading medical journals and adopted by some of the most prestigious institutions across the country. This role demands a passionate, empathetic individual who genuinely connects with older adults and is committed to their wellbeing. Working both in-person at our Gainesville location and virtually through our telehealth platform, you'll help shape our patients' wellness journeys through personalized coaching and compassionate support.
As part of our pioneering team, you’ll learn directly from renowned geriatric clinicians while having a front-row seat to healthcare innovation. This ground-floor opportunity positions you to make a meaningful difference in seniors' lives while gaining insights and experience that would typically take years to acquire in traditional healthcare settings.
Core Responsibilities:
Individual Health Coaching
- Conduct warm, supportive 60-minute health coaching sessions with senior patients both in-person and virtually
- Collect assigned quantitative and qualitative clinical data to inform our treatment algorithm using our digital tooling
- Design accessible, age-appropriate action plans using SMART goals
- Provide understanding, patient-centered support considering the unique needs of older adults
- Monitor progress and maintain detailed documentation
- Conduct proactive, caring outreach to maintain patient engagement
Senior Health Promotion & Community Integration
- Manage an active caseload of senior patients with empathy and attention
- Lead supportive, engaging health workshops and group coaching sessions
- Build meaningful relationships with local senior community resources
- Create and adapt programming to meet the needs of older adults
Integrated Care Team Collaboration
- Work closely with physicians, nurses, and specialists to coordinate patient care
- Lead initiatives in behavioral change and wellness coaching
- Participate in regular team huddles and/or care planning meetings
- Help bridge communication gaps between seniors and their healthcare providers

Required Qualifications:
-1-3 years of coaching experience in a health-related field experience
- Deep empathy for the seniors and their health related challenges
- Demonstrated ability to build trust and rapport with patients
- Outstanding listening skills and emotional intelligence
Preferred Qualifications
- Experience with EMR systems
- Bachelor's degree in health-related field
- Previous medical setting exposure (can include volunteer work, internships, or personal caregiving experience)
- Experience with Motivational Interviewing and behavior change models
- Experience working with senior populations
- Familiarity with zoom and similar platforms
-Innovation leaning: you enjoy trying and learning new things
Plus:
-National Board-Certified Health and Wellness Coach (NBC-HWC)
-Board certified Community Health Worker (CHW)
Physical Requirements:
- Manual dexterity for computer work
- Normal range of vision, hearing, and speech
- Capability to move through medical office setting
Compensation: $15-40 per hour
Variable based on qualifications and experience.
